Abstract

Introduction: The adherence of patients with hypertension with their medication is crucial for the attainment of optimal medication therapy targets. Pharmacists play a pivotal role in patient adherence. Objective: This study identified the barriers pharmacists in providing adherence support to patients in a public health center in Malang City. Methods: This study employed a quantitative descriptive method using an electronic survey. The instrument used involved a questionnaire comprising eight adherence barrier domains: pharmacist's knowledge, patient's understanding, pharmacist's concern, patient's awareness, facility for assessing adherence, pharmacist's ability in pharmaceutical care, medication management, and pharmacist-patient communication. The study sample is the entire targeted population, the pharmacists in Malang Municipal Public Health Center fulfilling the predetermined research criteria for one month from May 2021. Results: Fifteen pharmacists were involved in this study. The majority of respondents are female and aged ≤35. From the assessed eight domains, three primary barriers were found. The most prominent barriers faced by the pharmacists in improving adherence are communication with the patients (100,00%), facility limitation for assessing patient adherence (86,67%), and the patient’s limited understanding of treatment purpose and target (80,00%). Conclusion: The barriers experienced by the pharmacists in improving adherence are an actual condition of their work environment in the public health center. This correlates with the management of health care services from an external aspect. Therefore, collaborations between pharmacists, healthcare facilities, and other stakeholders are necessary to resolve these shortcomings Keywords: patient adherence; barriers; pharmacists; public health center
