Abstract
Healthcare-Associated Infections (HAIs) remain a critical global health challenge, particularly in ICUs, where multidrug-resistant organisms such as Carbapenem-Resistant Acinetobacter baumannii (CRAB) are frequently encountered. In mid-2024, a CRAB outbreak was identified in ICU of Ubaya Hospital (by Hermina). Objective To describe the infection control strategies implemented during CRAB outbreak, evaluate impacts & antibiotic usage. Methods Case study: Apr to Aug 2024. Data: ICU patients who underwent microbiological cultures & antibiotic susceptibility testing. Meropenem usage was quantified using Defined Daily Dose (DDD) measurements. Interventions included antibiotic rotation, enhanced hand hygiene monitoring, ICU environmental disinfection, patient cohorting, & strengthened collaboration between the Antimicrobial Stewardship Program (PPRA) & the Infection Prevention & Control team (PPI). Results During the outbreak peak in June 2024, seven CRAB isolates were identified, primarily from sputum samples. Following intervention, a notable decline in both CRAB isolation rates & meropenem usage was observed in July & August. The findings support the effectiveness of multifaceted strategies in curbing nosocomial CRAB infections. Conclusion A coordinated infection control response, integrating antimicrobial stewardship and environmental hygiene measures, contributed to the successful containment of a CRAB outbreak in a resource-limited ICU setting. Continued collaboration between PPRA and PPI teams is essential to sustain infection prevention efforts and reduce antimicrobial resistance in hospital environments.
