Abstract
Tuberculosis preventive treatment (TPT) is a key intervention in the global tuberculosis (TB) elimination strategy, as it reduces the risk of progression from latent TB infection (LTBI) to active disease, particularly among high-risk populations such as people living with HIV and household contacts of TB patients. This manuscript reviews current TPT regimens based on international guidelines (WHO, CDC, NICE, EU Standards, and the Canadian TB Standards), including longcourse isoniazid regimens (6H/9H) and shorter rifamycin-based regimens such as 3HP, 3HR, 4R, and 1HP, which generally demonstrate higher treatment completion rates. From a pharmacological perspective, isoniazid is effective but associated with risks of hepatotoxicity and peripheral neuropathy, whereas rifampicin and rifapentine may lead to clinically relevant drug– drug interactions, particularly with antiretroviral therapy. Evidence from cost-effectiveness analyses indicates that TPT is generally cost-saving and offers substantial public health value by preventing active TB cases and reducing long-term economic burden. Nevertheless, TPT implementation remains challenging due to limitations in ruling out active TB, gaps in identifying and reaching eligible populations, suboptimal adherence to long regimens, adverse drug reactions, complex drug interactions, and logistical barriers including supply chain constraints and inadequate service integration. Strengthening screening systems, ensuring access to shorter regimens, enhancing safety monitoring, and improving supply chain management and programmatic reporting are critical to expanding TPT coverage and maximizing its impact on TB elimination.
