Abstract

Background: Medication adherence is essential to successful pulmonary tuberculosis treatment. Previous reviews have often combined education with other adherence strategies, leaving its specific contribution uncertain. Objective: To synthesize evidence on structured health education and medication adherence and to appraise the methodological quality of the included studies. Methods: This PRISMA 2020 review searched PubMed and Scopus as primary databases and Google Scholar as a supplementary source for studies published from January 2015 to March 2025. Two reviewers independently screened studies and used design-specific JBI appraisal tools. Fifteen studies were synthesized narratively using SWiM guidance. Results: Counseling, printed and audiovisual materials, supportive education, and digital education generally improved adherence-related outcomes, particularly when interventions were repeated, culturally adapted, interactive, and linked to follow-up. However, most studies were small and non-randomized, used self-reported adherence, and had short follow-up. International trials also showed that digital monitoring alone may not improve clinical outcomes without timely individualized support. Conclusion: Structured health education is a useful component of adherence support but is not sufficient alone to ensure treatment completion. Programs should combine standardized education with individualized follow-up, adverse-effect management, and social support. Rigorous multicenter trials using standardized adherence and treatment outcomes are needed.
