Background Acute Coronary Syndromes (ACS) is one of the leading cause of death in Indonesia. ACS therapy require several drugs combination which increase the risk of patient experience Drug Related Problems (DRP). Clinical pharmacy practice can improve patient outcomes by detecting and solving DRPs. The impact of clinical pharmacy practice in patients with ACS has not been studied in Indonesia. The aim of this study is to measure the incidence of DRPs and to value the impact of the practice on total cost and Length of Stay (LOS) in patients with ACS. Methods A prospective study design was used in this study. Patients involved in this study were diagnosed by ACS and divided into two groups, control and treatment group. The study was conducted by reviewing medication and observing clinical condition. Data collected from this study then was analyzed using PCNE (Pharmaceutical Care Network Europe) classification system v 5.01. In the treatment group, clinical pharmacist made an intervention to solve DRP while in control group just simply observed. Outcome measured were an total cost and LOS. Outcomes of that two groups were analyzed using t-test. Results A total of 60 patients with ACS were admitted to Cardiology ward during 4 months period. 30 patients were in control group and 30 patients were in study group. 103 DRPs were recorded. The majority of DRPs referred to no drug prescribed but clear indication. Outcome measures were an overall costs and LOS. The average of LOS in control and treatment group was 8,6 and 5,3 respectively and significantly different (p= 0,05, CI 95%) .Meanwhile the average of overall drug cost in control and treatment group was significantly different (p= 0,05, CI 95%). Conclusions DRPs was common in patients with ACS. The most frequent DRP was untreated condition. The value of clinical pharmacist was proven by shorter LOS and total cost.
