Abstract
Introduction: Adverse drug event (ADE) is one contributing factor that associates with higher attributable health care expenditures. [1] ADE is also associated with polypharmacy. [2,3] Unfortunately, the true incidence of ADE is underreported. One of potential contributing factor of underreporting of ADE is lack of awareness. [4] Active monitoring is a startegy of early detection adverse drug reaction to ensure patient safety. [5] Aim: The aim of this study is to identify and describe ADE in hospital settings. Methods: This is a prospective observational study from January until April 2023 at a hospital in Surabaya. Drug utilization review (DUR) was done by a clinical pharmacology physician and a clinical pharmacist to identify ADE in inpatient settings. The identification of ADE was done by active monitoring of drug safety. Patient with polypharmacy was the entry point of active monitoring ADE. Results: From 347 patients, we identify 32 (9,2%) patients with ADE with 44 cases of ADE. We found the most ADE in April with 16 (37.2%) cases in 10 patients. During four months of active monitoring, hypokalemia is the most ADE with 11 (25,6%) cases, followed by bleeding 7 (16.3%) cases, thrombocytopenia 3 (7.0%), and constipation 3 (7.0%) cases. Atrial fibrillation manifested as a severe case of ADE in one patient. Conclusion: This collaborative practice could be a model to ensure patient safety in hospital by increasing awareness of ADE through active monitoring and early detection of ADE. Furthermore, it could be a potential strategy to reduce health care expenditure.
